Application Process

All shareholders are required to visit their respective broker to complete the By-Law 26 election process.

The process is as follows:

1. Complete and sign the By-Law 26 Election Form.

2. All registered holders must sign the Election Form where shares are jointly held.

3. Submit the completed Election Form and all required supporting documents to your broker no later than the election deadline.

4. Ensure your banking details are available to facilitate payment of the proceeds.

Application Requirements

Requirements for Individual (Personal) Clients

If you already have a brokerage account

Please submit the following to your broker:

  • Proof of banking account details.
  • One (1) valid government-issued photo identification for each account holder.
  • Signatures from all account holders (for jointly held accounts).

Requirements for Commercial / Corporate Clients

Commercial and corporate shareholders are required to submit the following:

  • Proof of banking account details.
  • List of Authorized Signatories and confirmation of signatories authority.
  • One (1) valid government-issued photo identification for each authorized signatory.

Requirements for Shareholders Holding Certificated Shares

For the purposes of the AGL By-Law 26 Offer, shareholders holding physical (certificated) PHL shares, including shareholders whose share certificates have been lost, must complete the certificate lodgement process before they are eligible to participate in the Offer.

Shareholders Holding Original Share Certificates

Shareholders who are in possession of their original share certificate(s) must:

  • Contact a registered broker to arrange for the lodgment and dematerialization of their share certificate(s).
  • Submit the required documentation to accept the Offer once the shares have been credited to their account in electronic form.

Shareholders with Lost Share Certificates

Shareholders whose share certificates have been lost, destroyed, or are otherwise unavailable must complete the share certificate replacement process before they can participate in the Offer.

This process includes:

  • Completing an Indemnity Form.
  • Having the Indemnity Form witnessed by a Commissioner of Affidavits or a Notary Public.
  • Publishing a notice of the lost share certificate in a daily newspaper for a period of twenty-four (24) hours.
  • Submitting the completed Indemnity Form and supporting documentation to their broker.
  • Arranging for the replacement certificate to be processed and dematerialized prior to submitting the Offer acceptance documentation.
